  4. Step 1 is an accordion. The seven specialty sections that used to sit lower down are gone, since the locations are now inside it. Bulleted lists do work in an accordion, so the locations are still bullets.

Steps 2 and 3 and the Pediatric Access Center fax numbers moved below the accordion so step 2 still reads onto the numbers it refers to. Three location links are dead here exactly as they are on the live page: Fetal Cardiovascular, Fetal Treatment and Radiology "Diagnostic" have no Refer page on the new site yet.

1. Check for special referral instructions.

Some locations have special referral instructions or forms. If you’re referring to one of the following specialties, open it below to see whether special instructions exist.

2. If special referral instructions don’t apply, use our standard referral form.

Most locations use our standard pediatric referral form (PDF). Complete it and fax it to the Pediatric Access Center at the appropriate number below.

3. Send additional materials, if applicable.

Send any relevant medical records, including test results and imaging, as well as authorization if required. Radiology imaging can be submitted electronically.
